Does Paying Cash Help at the ER? It Depends Who Owns the Hospital

If you're paying cash for an emergency-room visit in Virginia or North Carolina, which company owns the hospital can decide whether the posted cash price is a discount at all. In the price files each hospital is federally required to publish, every one of the 15 HCA hospitals we examined lists its cash price for a Level 5 ER visit at exactly its gross charge — no cash discount. Every one of the 4 LifePoint hospitals we examined lists cash at exactly 40% of gross — a uniform 60% discount.

First, the thing that matters more than any price: in an emergency, go to the nearest emergency room. Nothing below changes that. The billing conversation happens after you're safe.

The prices are for the facility fee of a Level 5 emergency visit — the most complex ER visit level, and the code hospitals list most consistently. Doctor fees are billed separately. Both ways of publishing a cash price are legal under the federal transparency rule; the point is that they are different, and you can't tell which one you're walking into from the outside of the building.

What this means if you're paying cash

A cash price equal to gross doesn't necessarily mean you'll be billed the full amount — it means the posted price carries no automatic discount. Three things worth doing before you pay any ER bill out of pocket:

• Ask billing directly: “What is your discounted cash price for this visit?” Get the answer in writing. At a hospital whose posted cash price is 40% of gross, this one question can be the difference of thousands of dollars.
• If the cash price equals the gross charge, ask about financial assistance next. Every nonprofit hospital — and many for-profits — has a financial-assistance policy with income thresholds, and it applies to emergency care. The hospital's profile page here lists what we know about its policy.
• If you have insurance but a high deductible, compare before assuming. At these hospitals, your plan's negotiated rate may be far below the posted cash price — the opposite of the pattern we've documented elsewhere. Check your plan's rate on the hospital's profile page.

Files freshly downloaded and re-derived July 25, 2026 (LifePoint files dated Q2–Q3 2026; HCA files current Azure postings); the same pattern was first measured April 29, 2026 with identical values. Scope: the 15 HCA and 4 LifePoint hospitals in Virginia and North Carolina whose files we examined — statements apply to those hospitals, not to either chain nationally.
